Healthwatch the consumer champion of health and care, has launched a special inquiry to find out what happens to people discharged from a health and care setting: focusing on three consumer groups – the homeless, those with mental health conditions and the elderly. They are reaching out to communities right across the country through the 148 local healthwatch. They also want to hear directly from organisations, the public, and front-line staff to understand the issues for these three groups, to learn what can be done to improve the discharge process for them, and examples of good practice. Healthwatch England will use the evidence collected, reporting on findings and use its powers to advise the secretary of state and the relevant national bodies, with recommended changes to policy, guidance or practice.
Organisations are invited to submit written evidence and public, front-line staff and organisations are also being asked to complete our on-line survey.
